To enable us to process Clinic
fees correctly, patients should complete the relevant
sections of the registration form and return it to
us as soon as possible, in the prepaid envelope provided.
Insured Patients
Patients who intend to use their medical
insurance should:

Contact insurers prior to admission
and quote the procedure code provided by the
Consultant. |

Obtain either a Claim Form, or a Pre-authorisation

Ensure that their policy will cover the treatment |

Where a Claim Form has to be completed, bring
it to Clinic on admission. |
If there is an excess in the policy,
this will be advised to the patient after the Clinic
has been paid by insurers and the patient is then
responsible for repayment of any such excess. Your
Consultant Surgeon and Anaesthetist will issue accounts
separately, patients should forward these to their
insurers.
Fixed Price Surgery
Fixed Price Surgery packages are offered
for a selected number of procedures. This facility
is available to self-funding patients. The Fixed Price
Package includes Clinic and Consultants’ fees.
Payment is required one week prior to admission. For

Self Paying Non-Fixed Price
Where the treatment is not on a Fixed Price
basis, self-paying patients can obtain approximate
costings from the Clinic. Payment of the Clinic account
is due on discharge from the Clinic. Consultants’
fees will be invoiced separately.
Payment by Sports Clubs/Associations
Where the patient’s treatment is to
be paid for by a sport club or other organisation,
the patient must obtain a letter of authority from
the payer accepting full responsibility for the cost
of the treatment.
